Thank you for your interest in becoming a Fork Over Love Certified Restaurant Partner. Together we are creating a new system of nourishment for our entire community by bridging the gap between small, independent restaurants and residents.

Please complete this application. You will be asked to upload a copy of the following, please gather your materials prior to completing the form:
- W9 (signed and dated)
- Most recent health inspection
- Current menu
You will receive an email regarding your application status within five business days, at which time we will review restaurant certification requirements with you. If you do not receive notification of acceptance or denial within five days, please email us at info@forkoverlove.org.
